Being Underweight Shown To Have
Similar Health Risks as Being Obese

As the prevalence of obesity increases, the concern for increased risk of death also increases. Although numerous studies have documented an increased risk of death for those who are obese and overweight, the majority of those studies did not however fully adjust for other factors such as age and smoking. These adjunct factors may have been responsible for some of the increase in mortality rates in the overweight and obese populations studied. The purpose of this study was to use a different statistical model, which adjusts for other factors to estimate deaths associated with underweight, overweight, and obesity in the United States in 2000.

Using data from the National Health and Nutrition Examination Surveys I, II, and III (NHANES ) the relative risk of mortality was assessed with different levels of body mass index (BMI). Three levels of BMI were studied: underweight (BMI less than 18.5), overweight (BMI 25 to 29), and obesity (BMI 30 and above).

Relative to the normal BMI category (19 - 24), obesity was associated with 111,909 excess deaths while being underweight was associated with 33,746 excess deaths. With the absence of other factors, being overweight alone was not associated with excess mortality.

The results of this analysis indicate an increased mortality relative to the normal weight category in those in the obese and underweight categories. The results also show a decrease over time in mortality due to obesity. This decrease may be due in part to improvements in public health and medical care and the increases in life expectancy in the US along with declining mortality rates for heart disease.

Fitness Professionals should point out to their clients that this study does not in any way minimize the importance of maintaining a healthy weight. The results of this study only show no increase in mortality rates for those who are overweight. It does not address quality of life or incidence of chronic disease. Due to medical advances we are seeing a wider distance between a person’s life span (the number of years they are alive) and their health span (the number of years they are functional and free of disease).

to Increase Health Benefits

Current physical activity statistics show that the majority of adults do not perform enough physical activity to maintain or promote good health. One of the greatest challenges to Fitness Professionals is to find activity programs that are realistic and motivational to this sedentary population. The purpose of this study was compare the number of steps accumulated by women instructed to walk 10,000 steps per day with those told to take a brisk 30-minute walk.

Fifty-eight sedentary women served as subjects and were randomly assigned to either a 10,000-step group or a 30 minute walk group. Subjects wore a sealed pedometer to establish baseline physical activity and to keep track of their steps during the 4-week study period.

Both groups reached their assigned walking goals only 4 times per week. The 10,000 steps group averaged 10,159 steps and the 30-minute group averaged 8270 steps. On days when they did not meet their walking goals the 10,000 steps group averaged 7780 steps and the 30-minute group averaged 5597 steps.

Fitness Professionals can use the results of this study to increase awareness of the importance of daily physical activity in their clients, even on days when the clients cannot “squeeze in” a 30 minute walk. Using a pedometer and assigning a specific number of steps is a viable way to increase a client’s physical activity on both exercise and non exercise days. For many Fitness Professionals the rallying cry is “10k per day!” to increase awareness of the importance of daily activity.
